Well I think it's because many of the Bayformers designs are so over complicated that a lot of people missed it.

Or that since many many images of Ironhide had two normal eyes up to that point, our minds simply didn't pay it much notice since it's such a relatively minor part of his look.
I give Ratchet's inability to heal BB's vocal processor (after centuries of trying, lol, at least he's persistent.) a pass. The implication is that the damage was so severe it's relatively unfixable; much like a human suffering a spinal injury. However, I would like to see Ratchet successfully heal at least one bot in TF2.

The damaged eye is easy to miss because it's not prominent. It blends in with the rest of IH's face extremely well, like the cowboy hat. It's not there to draw attention. It's as if the designers are saying "that's just the way it's suppose to be".
I saw a little bit of damage OVER the eye in TF1. But this is new. It's like, he got puched in the face or sumtin'...
ok just watched tf1 on a blu-ray big screen, you can see it fairly well just after ironhide flips over the screaming woman in mission city
