My LG Dai Atlas had 2 left hands, I did manage to get the arm swapped out from where I bought it.
My Iron Factory Helex came with two lower right arms. Fortunately that toy is something of a parts-former (for the DJD combiner effect, anyway), so I got a replacement lower arm from the Australian vendor after they made a trip to China.
I think the only instance of this I have ever experienced is a POTP Elita with the swapped feet.
I forgot about this, it was like the whole first run had this problem didn't it?
You might be right about that...I recall there were a bunch of them that were jacked up.
The original that i can remember is Cybertron Ransack with the back wheels being flipped
My Nemesis Pax was missing a fist out of the box. I had the same situation where Amazon was out of stock, but I never was able to catch it back in stock and get a replacement. It still bugs me to this day cause it really was a great looking figure from POTP. It's like eating a really good burger and 3 bites in you find a dead bug under the bun. Ruins the whole thing for me.

His right arm had a spring-loaded forearm piece that was meant for his left arm (his left arm was fine). I had to remove the panel and reshape it with a file in order for it to fit the grooves correctly and be able to compress for transformation.
I don't know how to describe this well but his foot is mainly diecast but with a plastic circular piece that sits in a groove on one side. But on one foot, the plastic piece is on the wrong side of the foot so it's creating incorrect tension and won't close correctly for transformation.

Elita was an easy fix. I never got around to fixing Ransack. I need to make time to take pins out and separate those figures that need it. My TR Hot Rods is another one. The canopy doesn’t quite close. I suspect that the pin is slightly crooked.
I tried ransack and broke the plastic. The wheel still holds in leg mold but its not great.
My RID2015 Warrior Class Starscream came with two left hands.
Not sure if this counts but the electronics on my tr fort max didn't work out of the box
