Maybe it's just because I'm watching it now, but the skinny-dipper at the beginning of Jaws may have experienced the most miserable and undesirable death I've ever seen in a movie.
I feel like I could deal with almost any land death at the hands of humans - even cannibals - better than I could having to avoid drowning long enough to be devoured by a gigantic fish who's turning the tables on you from your last trip to Red Lobster.

For the movie Buried Spoiler Ryan Reynolds character. The whole film focuses on him. You have hope for him making it and it ends with him dying being buried alive. It’s a death that hurts you in your soul.
That was just an agonizing movie to watch at all. Being stuck for hours on end in a coffin is a shitty thought.
Fucking toxic waste… Obviously. Jesus, that one was bad. The actor actually gasped for air too, like he was suffocating from having his alveoli scalded.
There was a kung fu movie that unfortunately I don't know the name of... Edit: I googled it "Five Elements Ninjas" (1982) Ninjas kept tunneling under ground stabbing up a some guy, he would try leap away but they still got him bad. When the leader of the ninjas appeared the guy was so messed up, he stepped forward and tripped on his OWN INTESTINES. The ninjas then finished him off.
It’s small potatoes compared to what’s mentioned here but Lacroix’s sabotaged execution by electric chair in the Green Mile was pretty fuckin’ unpleasant. Also as a rule, drowning in a movie will do my head in. So that’s like, 30% of the deaths in Titanic and a more directly affecting one is Vesper Lynd in Casino Royale.
Cohaagen suffocating to death at the end of Total Recall. It was so drawn out, I actually changed the channel before it was over.
So, this is one which actually happened and must’ve involved a level of pain that is incomprehensible. Cecilia Ann Shepherd was murdered in 1969 via repeated stabbing from a bayonet. She was stabbed in the back, abdomen, lungs, neck, genitals, etc. Ten times, in total. She then had to wait over an hour for her injured boyfriend to flag down help and then for an ambulance to arrive… She was conscious the whole time, begging the paramedics to either give her painkillers or to just kill her. She died that night while in surgery. I’ve been stabbed in the arm once. It hurts. Knowing that this deeply puritanical, Christian kid was begging to either be given drugs or euthanized makes it really funny whenever some paper tiger goes on and on about how they could handle anything without pain relief. If you think that, it’s because you haven’t been in enough pain.
Bone Tomahawk is a tremendous contender, but at number 2 I'd put Open Water due to the psychological component.
