The neck piece I was thinking of would be something really simple. Basically, it could lengthen or shorten by sliding into the head. that way it could stretch up to clear the shoulders like in the pictures. OR it could slide in or out of the body (if there's a space for it) and the head end could be topped with a ball joint for the head.
The chest on mine seems to stay in position pretty well, though if you give it a push, it will snap back down. If it's a problem, there's probably a small fix for it. Maybe just adding a little piece of plastic that prevents the chest from flattening.
As for the arm clearance issue, in my picture, I elected not to lock the shoulders in place because the narrower silhouette looks a bit more feminine. However the hinge joint at the shoulder (by the wheel) is still there so the original range of movement remains... if she needs to take a pose with a raised arm, the arm can still slip out from behind the chest pretty easily.
