    Okay, so I went to the Achievements and cataloged all XIII chapters, so here we go:

    The Exodus: Bumblebee
    Defend the Ark and Metroplex Heeds the Call: Optimus
    Eye of the Storm: Cliffjumper and Jazz
    Cut and Run: Jazz and Swindle, possibly Swindle/Onslaught or Swindle/Brawl
    Death From Above: Vortex
    Belly of the Beast: Shockwave and/or Soundwave
    Combaticons, Combine!: Self-explanatory
    Bruticus, if you feel like being cheap and not guessing
    Megatron Returns: In the title
    The Final Countdown: Possibly Wheeljack
    Starscream's Betrayal: In the title
    Grimlock, SMASH!: In title
    'Till All Are One: Jetfire, Prime, Megatron

    12 levels? Sweet, I hope each of them are at least 30 minutes long. I want to play the hell out of this game.

    How do we know Jetfires in the last level ? I thought the achievement was just for either prime or megatron
