You'd be surprised how well an "obedience under the threat of extreme violence or death by your superiors" program works on members like that. Basically the Decepticons (historically) rule through fear and oppression, and for some unstable units they represent the only place where an outlet for anti-social or homicidal behavior exists. In some cases, however, units can be corralled or confined until battle, and then just sent out in the general direction of the enemy, like a berzerker. In extreme cases, unstable members can be confined, placed in stasis, or executed, if their instability is considered a sufficient hindrance to the agenda of the Decepticons.

I imagine some of the worst ones to be treated as less than individuals and more like animals. Keep 'em in a cage, feed them nice aggressive videos to keep their rage up, then drop them in the middle of the Autobot army. Instant berserk beast rampage.

Or possibly like Monstructor has, a control that shuts them off if they get out of control. Like Rampage in Beast Wars was controlled by Megatron (and later Dinobot II) having a piece of his Spark in a torture matrix.

Though I'm no fan of IDW, their Megatron was no idiot, he had off switches and blind spots programmed into some of his more dangerous bots.

In BW, To keep Rampage in line Megatron had his spark kept in a energon crystal and when he disobeyed orders he'd crush it so Rampage would be kept in line.

That and Mind Control helps as well.
